Adoption Application

Please read before filling out application:

The application must be completely filled out before being submitted. 

All resident pets MUST be spayed/neutered, up-to-date on vaccinations and on flea/tick and heartworm preventative

If you are not serious about adopting a Mastiff from GPMR please do NOT fill out an adoption application.  We are all volunteers and it takes alot of time to process an application. Please check with your spouse and/or roommate PRIOR to filling out this application. Please be sure you have around 10 minutes and access to contact information so that you can complete the application.Thank you.
